    :: Home Health Aide certificate and Checklist from an accredited school in the District of Columbia
    :: Proof of Eligibility to Work in the United States

    :: Current Physical Clearance from a physician

    :: Current CPR certification (from AHA or Red Cross)
    :: Criminal Background check conducted within forty five (45) days, from The District of Columbia and all other States lived in, during the last seven (7) years)
    :: Valid Picture Identification

  • Physical Therapist and Occupational Therapist

    :: Complete work History

    :: Proof of Professional Liability insurance
    :: Valid Picture Identification
